CARROLL COUNTY WELLNESS TRANSIT 

CCWT is designed to improve the overall wellness of our community.  Our mission is to meet the transportation needs of those in our community who currently have no other transportation resource available to them.   CCWT dispatch will take responsibility of scheduling that transportation and the client will be notified of the appropriate vendor to handle their transportation needs.

Language assistance is provided at no additional cost.

Scheduling Requirements:

•  Pick-up location must be in Carroll County, KY.
• Transportation provided at no cost.
• Physical Address and Working Phone Number- Dispatch MUST be able to confirm pick-up time and location within 24 hours of appointment time.
• ALL appointments must be scheduled 72-business hours in advance.
• Additional operating hours may be available in special circumstances depending on staffing capabilities.
