Requesting Records from CCMH

You can request your personal medical records through our Health Information Management (HIM) Office.

In Person

Visit the HIM Office at 309 11th Street, Carrollton, KY 41008.
Please bring a driver’s license or state-issued ID and complete a Medical Records Request form.

By Fax

For continuity of care, you may fax a request to 502-732-3226.
