Insurance Verification
This facility will bill your primary and secondary insurance carriers or governmental agency directly for facility fees. Please bring your most current primary and secondary insurance card, Medicare card, or public assistance card with you on the day you receive hospital services.
Patients should be familiar with their insurance plan’s admission and coverage requirements. In some cases, pre-authorization, physician referral, or a second opinion may be required to ensure maximum reimbursement. Failure to meet these requirements may reduce or eliminate insurance benefits.
Billing Process
Billing statements are submitted to your insurance carrier shortly after health care services have been rendered. Once your insurance company processes the claim, you will receive an Explanation of Benefits (EOB).
The EOB is not a bill. It explains how much your insurance company has paid and the portion that is your responsibility. Carroll County Memorial Hospital will then send you a bill for any remaining balance owed. Payment is due upon receipt.

Please note that you may receive multiple bills. The hospital bill covers facility charges only and does not include professional fees for services such as laboratory work, pathology, medical care, surgery, or anesthesia. These services are provided by independent practitioners who will bill separately.
Payment
Patients should be prepared to pay any required co-payments or deductibles at the time services are rendered. Patients without insurance coverage or those receiving non-emergent services may be required to pay their co-pay or deductible on the date of service.
Patient bills may be paid by check in person, dropped off at the hospital, mailed, or paid online through our patient payment portal. Payment is due upon receipt of the hospital bill.
