At Carroll County Memorial Hospital, our Transitional Care program provides short-term rehabilitation and skilled nursing for patients who need additional support before returning home. Also known as our swing bed program, this care helps you regain strength and independence in a comfortable setting close to home.

 

Who Can Benefit?

Transitional Care may be right for patients recovering from surgery, illness, injury, or a hospital stay who still need skilled nursing or rehabilitation before they can safely return home. Our team works with each patient to create a personalized care plan that supports their recovery goals.
